[olug] Visitor Sign In

Hey all

I am looking for a simple to use and simple to configure visitor sign-in
system.
So I thought I would poke my linux brethren.   A flat text file to store
the info
is great, interface with gdocs is a possibility.

I have a very limited amount of time to put it together.  I think it should
be able to
collect Name, email, phone, interest level.  Basic idea:

   - welcome
   - sign-in
   - next
