Since /home isn't managed by LVM it is far simpler to:

umount /home
setup partitions on the 2 drives the way you need them. Make sure you
set the FS type to raid autodetect
mdadm --create /dev/md0 ...
mkfs.ext4 /dev/md0
edit /etc/fstab to reflect new /home
mount /home
