I've used mutt for quite a long time.  I still like it, but have slowly
been moving over to Mahogany (http://mahogany.sourceforge.net).  Right
now, I'm testing the new 0.62-prerelease - it's pretty snazzy.  

I'm moving over to it because of:
 o tighter intergration of multiple mailboxes
 o news reader
 o Python scriptable
 o Palm syncing abilities
 o easier multi-mailbox handling

But Mutt is fast, ultra-configurable, but I found that once I saved messages
into other mailboxes, I never read them again.  I would recomment it over
elm or even pine.  

If you want gui-based, check out Mahogany, but wait until 0.62 - 0.60 has
a few bugs.  And the developers keep saying that 0.62 will be "out in a
few days" for weeks now, so I guess that it means that it will be out
Real Soon Now.
